How to write an incident report?

An incident report must have the complete details of the incident. It narrates all the events surrounding the incident in question in their exact manner of occurrence. It must include the following important things like the circumstances surrounding the incident, Date, time, location, witnesses, and the victims. After-which, Corrective action shall be taken to reduce the likelihood of another.
